A fire engineer gets a visit record, a defects list and a customer email. A physio gets a clinical note, a GP letter and an exercise sheet. Each trade gets its own set.
Every record is read back against what you actually said, looking for anything added: a number, a side, a date, a claim. Measured, not promised — the figures are on the front page.
What you didn't say is marked [not stated] and raised as a short question for the person who was there. It is never filled in with something that sounds likely.
